A Note About Sponsorship
Each person ordering from Simplexity Health must have a "sponsor." When you checkout after placing your order, you will be asked to provide the name or ID# of your preferred sponsor. If we cannot locate your preferred sponsor because they are no longer active (or if their ID is invalid) you will be signed up under another qualified sponsor. Also, if you do not order in 6 months or you do not indicate a sponsor, you may be signed up under a different sponsor on your next order. Nothing else will change.
